Complete Rehab Pathway

Through our Hospital, homes and other specialist facilities,
patients benefit from a consistent and complete pathway of
care in one location from initial assessment and structured
rehabilitation programmes, through to pre-community and
supported living.

Our complete pathway allows us to accept patients at different stages
of rehabilitation.  As patients progress at Glenside they move along
one or more of our three rehabilitation pathways as part of a
tailored programme, not in a pre-determined linear fashion.

Our rehabilitation pathway consists of three ‘routes’ and offers
rehabilitation and medical care in hospital and non-hospital settings:

• Neuro Rehabilitation Pathway (Main Pathway)
• Neuro Behavioural Pathway
• Complex Care Pathway (Intensive Care & High Dependency Care)

The goal of the rehabilitation programmes across our five Hospital wards
is to prepare patients for discharge from the Hospital to other homes in our 
pathway, including community-based rehabilitation activities.

All treatment plans are designed under the direction of a Consultant
in Rehabilitation Medicine.

Rehabilitation Journey

Patients usually begin their rehabilitation journey in our main pathway assessment ward (Avon), when at the acute/stable stage.

Depending on their needs and level of progress, patients may move between pathways.  As they improve and increase their independence, they are discharged from the Hospital into another home on our campus to continue with their rehabilitation and care programme.  Some patients move between homes in our pathway until they are able to trial independent living in one of our ten supported living bungalows to develop their social and domestic skills.

We don’t work to fixed timescales.  Patients can start at any point in the pathway and move between them depending on their individual needs and level of progress.
