Supported Living

Horizon Close simulates independent supported living via ten purpose-built bungalows, each with their own front door.

Ranging from short to long-term stays, Horizon Close represents the final stage in our complete rehabilitation pathway.

Rehabilitation and therapy teams provide personalised care and develop strategies to help service users compensate for the consequences of their brain damage/neurological condition and live as independently as possible.  Vocational support is also provided to aid transition back into the community.

The bungalows are self-contained with a fully equipped kitchen, living room, and en-suite bedroom.  Residents can choose to prepare their own meals in the bungalow or request a meals-on-wheels service from the campus catering service.  Individual nurse call systems enable residents to contact staff for assistance, 24 hours a day from each bungalow. 

The Close simulates community supported living with simultaneous support to prepare the service user for discharge to a community placement.  Alternatively, it may be a long-term placement for service users who would present a risk in the community but wish to live as independently as possible.
