Horizon Close consists of ten bungalows, nine for supported independent living and one for assessments. They are used to accommodate people who are more independent or at the final stages in the rehabilitation pathway. Horizon Close is registered as a Residential Care Home with the Commission for Social Care Inspection.
The team at Horizon Close provides social care, support and on-going rehabilitation to adults with long-term neurological conditions including brain injury. The team use the bungalows to assess and support the residents prior to returning to the community.
The bungalows are self contained and fully equipped with a kitchen, living room, and en-suite bedroom. Each resident has an individual assessment and appropriate risk assessments. Staff are available 24 hours a day and a nurse call system ensures that staff can be contacted at all times from each bungalow. Main meals can either be prepared in the bungalow or delivered as a meals-on-wheels service.
Each resident’s rehabilitation programme is individually designed to meet their needs. They may benefit from Physiotherapy (with hydrotherapy in the pool), Occupational Therapy, Speech and Language Therapy, Clinical Psychology and Cognitive Rehabilitation as appropriate. Recreational assistants support the team in providing social activities and help the residents with tasks such as shopping trips.
