The philosophy of the Fast Track Rehabilitation Environments, Grovely House and Newlands is to provide a homely environment whereby Service Users are empowered to make their own decisions regarding life choices and other areas of daily living. Based on social rehabilitation models, the Homes offer short to medium term rehabilitation placements, (twelve to eighteen months), in a supported therapeutic environment.
Following care and social assessments by the inter-disciplinary team, Service Users are involved in developing their own plan of care, looking at both rehabilitation and social perspectives. Staff facilitate Service Users to lead as normal a life as possible, continually encouraging and developing each Service User in an attempt to meet and maintain their own individual potential. A sense of both responsibility and self worth can be fostered within this framework and aims to maximise independence, increase community integration, build confidence and to enable the transition from a care environment towards living independently. Rehabilitation programmes are monitored by the Home Managers and other lead members to the inter-disciplinary team, with the emphasis being placed on enabling Service Users to become equipped for living within an independent or supported community living environment.
Newlands & Grovely House Newlands opened in 1998 and is a nine bed Home, and Grovely House opened in March 2001 and has 7 beds. Both Homes are registered with the Commission for Social Care Inspection for Social Care for adults with an acquired or traumatic brain injury aged 18 -55.
Preparation to transfer to Transitional Living Assessment The end goal of this phase is to prepare the Service User for admission to the transitional living assessment area in preparation for a community discharge. Service Users who are unable to meet the criteria for transfer to the Transitional Living Environment following this Fast Track rehabilitation placement are either offered a placementon our long stay continuing care unit, or are recommended for transfer to a similar long stay environment off site
