Following assessment of needs, Service Users staying within our services are transferred to the intensive rehabilitation phase of their placement for the commencement of intensive rehabilitation. The general ethos of rehabilitation is based on a social rehabilitation model insorporating cognitive rehabilitation therapy. The average length of stay with the intensive rehabilitation phase is expected to be between three and nine months, depending on the needs of the individual Service User.
Goals are established in conjunction with the Service User, their significant others and members of the inter-disciplinary team to ensure a Service User centered approachto rehabilitation and care delivery. Progress through the service pathway is reviewed weekly at inter-disciplinary communication meetings, and formally at regular revies where all relevant members of the team, including care managers and family are invited to attend.
Preparation to transer to Fast Track Rehabilitation The end goal of this phase is to prepare the Service User for admissionto Grovely House or Newlands, small six and nine bed homes, where rehabilitation can continue in a more community-style setting, in anticipation of a future discharge to community supported or independent living.
Service Users who are unable to meet the criteria for transfer to our Fast Track Rehabilitation Environments following their intensive rehabilitation placement are either offered a placement on our long stay continuing care unit, or are recommended for transfer to a similar long stay environment off site.
