Referral

Prior to admission, we undertake an assessment of each prospective patient to ensure that rehabilitation, nursing, medical and social needs can be met. We require copies of previous reports, medical notes and scans as well as talking to those presently involved in the patient’s care: medical professionals, care/case managers and family members.

We actively encourage prospective patients (if possible) and their family members to visit us prior to admission. You will be given a tour of the campus and facilities, and be able to meet members of our MDT, who provide our care and rehabilitation services.
