Pathways
Patient admission and discharge is possible at any stage of our pathways. Due to Glenside Care's unique needs-led service, patients often move between pathways, based on their individual needs.
The main pathway focuses on neuro rehabilitation and is provided at both Glenside Hospital in Salisbury and Glenside Farnborough, opening in May 2012. At Glenside Hospital there are several wards and homes within the pathway, which each provide specialist support and services for people at various stages of rehabilitation. They range from Avon Ward, where initial assessment and treatment often takes place, to Bourne Ward, where intensive rehabilitation takes place and Newlands, which provides short-term or medium-term stay for neuro rehabilitation. Horizon Close also comes under the main pathway umbrella and offers pre-discharge support in a simulated supported living environment. The main pathway extends into the community as it continues through Glenside Farnborough (opening May 2012), where a range of accommodation encourages patients further along in their rehabilitation to regain the skills they need to return to independent living.
The neuro behavioural pathway is specifically designed to help those with challenging behaviour as a result of a neurological condition. Assessment and initial treatment can start at Ebble Ward, where there is a staff to patient ratio of 1:1, however, patients can enter the pathway at any stage depending on their individual circumstances. Services within the neuro behavioural pathway include Nadder Ward and Old Vicarage and Limetree facilities, which focus on supporting patients with mild to moderate challenging behaviour to develop socially acceptable behaviour strategies through a variety of activities and positive behavioural support.
Glenside Hospital in Salisbury also provides the complex care pathway. Within this is the Wylye Ward is our level 3 intensive care facility, allowing us to take patients directly from an NHS intensive care unit who are either ventilator-dependent or require tracheostomy weaning. Patients with complex needs, including Minimally Conscious and Persistent Vegetative State can also be admitted to our Pembroke Lodge or Newton House facilities for longer term high dependency care.
Patients can move along or even between pathways at various stages of their rehabilitation and their progress is constantly monitored by our MDT to ensure they are in the best environment to suit their needs.
