Main

For many patients on the main pathway, the rehabilitation journey begins on Avon Ward for assessment and diagnosis of long-term neurological conditions, including acquired/ traumatic brain injury and post-traumatic amnesia.

However, patients can be admitted to Glenside at any stage of their rehabilitation and can come into the main pathway at any point.

Bourne Ward lies within the main pathway and provides an intensive multi-disciplinary rehabilitation programme. Focusing on patient-centred goals, including our Cognitive Rehabilitation Therapy (CRT) Programme, rehabilitation aims to strengthen and establish structured patterns of functioning to compensate for impaired neurological systems.

Patients can also come straight to Newlands, a short-/medium-term stay home for patients in a group living environment where the focus is on community, social and vocational rehabilitation. This provides a homely environment, with support from rehabilitation assistants and therapists, and is designed to move patients towards increased independence to transfer to a long-stay environment or supported living in the community.

Horizon Close then offers short-/long-term simulated supported living in ten purpose-built bungalows, each with their own front door. Rehabilitation and therapy teams provide different levels of personalised and bespoke care to enable patients to live as independently as possible in preparation for patient discharge. To complement this, the Horizon Centre provides a base for individual therapy programmes and social activities in the evenings and weekends.
