Referrals

We welcome general enquiries about our services and new patient referrals from family members, healthcare and other professionals.

For all general enquiries please complete our enquiry form.

Family Members
Prior to admission, we undertake an assessment of each prospective patient to ensure that rehabilitation, nursing, medical and social needs can be met.  A pre-assessment questionnaire should also be completed.

We actively encourage prospective patients (if possible) and their family members to visit us prior to admission.  You will be given a tour of the campus and facilities and be able to meet members of our multi-disciplinary team who provide our care and rehabilitation services.

Healthcare and Other Professionals
If you are unfamiliar with Glenside, you are welcome to organise an informal visit to view our facilities and meet senior members of our rehabilitation team.  If you wish to make a specific referral, please email Margaret Hiscock our Director of Nursing to discuss your patient’s specific requirements.

We are a pre-authorised provider with numerous Primary Care Trusts.  If you feel that our expertise matches your patient’s care and rehabilitation needs, we can normally arrange for them to be assessed within 48 hours and, if suitable and subject to approval from the relevant funding authority and occupancy levels, we can arrange for immediate admission.

Members of our multi-disciplinary team are experienced in providing medico-legal assessments for personal injury claims and other investigations.


Pre-Assessment Questionnaire
We require that a pre-assessment questionnaire is completed for all prospective referrals:

In order that an assessment is as comprehensive as possible, we usually also require copies of previous reports, medical notes and scans.  We may also contact those presently involved in the patient’s care; medical professionals, care/case managers and family members.

Funding
Our services can be funded by the NHS, Social Services or a private medical insurance company (subject to policy restrictions).  Self-funding is also an option.  As part of the pre-admission assessment we look at the type and amount of funding required, based on a patient’s needs.

Prior to admission for patients that are accepted for treatment, we require written approval from the appropriate funding party.  Admissions are also subject to bed availability.
